Aix\u00f2 vol dir que entre 1 i 2 de cada 4 persones pateix de pressi\u00f3 alta, encara que no sempre ho s\u00e0piguen. <\/strong><\/h1>\n<p style=\"text-align: justify;\">I potser, tampoc no coneguin o no siguin conscients, que igual que la diabetis mellitus, la hipertensi\u00f3 arterial (HTA) \u00e9s un dels principals \u201cassassins silenciosos\u201d que afecten la nostra societat. Moltes vegades no donen s\u00edmptomes, per\u00f2 de mica en mica va danyant els nostres vasos sanguinis i els nostres \u00f2rgans interns, augmentant el risc de complicacions greus i potencialment mortals. Cada mil\u00b7l\u00edmetre de mercuri (unitats en qu\u00e8 mesurem la pressi\u00f3 arterial) que es controla \u00e9s temps guanyat i salut preservada, s&#8217;anima a con\u00e8ixer perqu\u00e8 aix\u00f2 \u00e9s aix\u00ed?  <\/p>\n<p style=\"text-align: justify;\"><em> Dra. Minguito &#8211; Equip M\u00e8dic Neolife <\/em><\/p>\n<hr>\n<p style=\"text-align: justify;\"><strong>Qu\u00e8 \u00e9s l&#8217; hipertensi\u00f3 arterial?<\/strong><\/p>\n<p style=\"text-align: justify;\"><strong>L&#8217; hipertensi\u00f3 arterial \u00e9s un dels principals riscos cardiovasculars de la nostra societat. Des de Neolife volem donar-los una perspectiva p<\/strong>erqu\u00e8 entenguin qu\u00e8 \u00e9s i per qu\u00e8 \u00e9s tan important tenir-la ben controlada aix\u00ed com el que suposa el mal control. Aqu\u00ed hi ha un spoiler: si li importa el cervell, el cor i els ronyons, necessita una pressi\u00f3 arterial baixa.  <\/p>\n<p style=\"text-align: justify;\">Quan parlem de pressi\u00f3 arterial, ens referim a la for\u00e7a que exerceix la sang contra les parets de les art\u00e8ries mentre el cor batega. Tots hem sentit, quan anem al metge, dos mesuraments: l&#8217;alta i la baixa, per\u00f2 a qu\u00e8 ens referim realment amb cadascuna? <\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter wp-image-1057 size-large\" src=\"https:\/\/www.neolifesalud.com\/wp-content\/uploads\/NL-presion-arterial-0.jpg\" alt=\"presi\u00f3 arterial\" width=\"1024\" height=\"683\"><\/p>\n<p style=\"text-align: justify;\">El cicle card\u00edac t\u00e9 dues fases principals: la s\u00edstole i la di\u00e0stole. A la s\u00edstole, els ventricles del cor, que s\u00f3n les c\u00e0meres inferiors, es contrauen per expulsar la sang. El ventricle esquerre \u00e9s especialment important en aquest proc\u00e9s, ja que \u00e9s m\u00e9s muscul\u00f3s que el dret, i la seva funci\u00f3 \u00e9s bombar la sang cap a tot el cos, superant la resist\u00e8ncia del sistema circulatori perif\u00e8ric. Val a dir que, en aquesta explicaci\u00f3, ens estem referint a la circulaci\u00f3 sist\u00e8mica o perif\u00e8rica, \u00e9s a dir, la que irriga el cos en general, i no a la pressi\u00f3 pulmonar, que \u00e9s un mesurament diferent. La pressi\u00f3 arterial, que comunament es reporta com a &#8220;120 sobre 80&#8221;, per exemple, es refereix a la pressi\u00f3 en la circulaci\u00f3 perif\u00e8rica. La pressi\u00f3 als pulmons, que \u00e9s controlada fonamentalment pel ventricle dret, \u00e9s considerablement m\u00e9s baixa.     <\/p>\n<p style=\"text-align: justify;\">Durant la s\u00edstole, el ventricle esquerre es contrau i empeny la sang cap a fora del cor a trav\u00e9s de la v\u00e0lvula a\u00f2rtica, entrant a l&#8217;aorta (la gran art\u00e8ria que surt del cor) i es distribueix r\u00e0pidament per tot el cos. En aquesta fase, la sang exerceix pressi\u00f3 contra les parets arterials, cosa que constitueix la pressi\u00f3 sist\u00f2lica (la tensi\u00f3 \u201calta\u201d). <\/p>\n<p style=\"text-align: justify;\">La segona fase del cicle \u00e9s la di\u00e0stole, que \u00e9s igualment important. En aquest moment, el cor es relaxa despr\u00e9s d&#8217;haver expulsat la sang, permetent que les c\u00e0meres ventriculars s&#8217;omplin novament des de les aur\u00edcules. \u00c9s en aquesta fase quan el cor mateix rep el seu reg sanguini a trav\u00e9s de les art\u00e8ries coron\u00e0ries. Encara que la pressi\u00f3 arterial \u00e9s m\u00e9s baixa durant la di\u00e0stole, segueix havent-hi una pressi\u00f3 constant o t\u00f2nica dins de les art\u00e8ries (pressi\u00f3 di\u00e0stolica, la tensi\u00f3 \u201cbaixa\u201d).   <\/p>\n<p style=\"text-align: justify;\"><strong>Qu\u00e8 vol dir tenir la pressi\u00f3 arterial alta i per qu\u00e8 \u00e9s tan important mantenir-la en nivells \u00f2ptims?<\/strong><\/p>\n<p style=\"text-align: justify;\">Des del 2017, les guies han canviat, despr\u00e9s de la publicaci\u00f3 de l&#8217;assaig cl\u00ednic SPRINT (Systolic Blood Pressure Intervention Trial) el 2015. Les recomanacions actuals, en vigor des de fa aproximadament sis anys, estableixen el seg\u00fcent: <\/p>\n<p style=\"text-align: center;\"><strong>Classificaci\u00f3 de la pressi\u00f3 arterial (segons guies actuals)<\/strong><\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-1057 size-large\" src=\"https:\/\/www.neolifesalud.com\/wp-content\/uploads\/NL-presion-arterial-2.png\" alt=\"pressi\u00f3 arterial\" width=\"1024\" height=\"683\"><\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-1057 size-large\" src=\"https:\/\/www.neolifesalud.com\/wp-content\/uploads\/NL-presion-arterial-1.png\" alt=\"pressi\u00f3 arterial\" width=\"1024\" height=\"683\"><\/p>\n<p style=\"text-align: justify;\">Per exemple, una persona amb pressi\u00f3 de 120\/83 mmHg ja entraria a la categoria d&#8217;hipertensi\u00f3 etapa 1, encara que la seva pressi\u00f3 sist\u00f2lica encara estigui dins del rang normal.<\/p>\n<p style=\"text-align: justify;\"><strong>D&#8217;on provenen aquests valors?: l&#8217;assaig SPRINT (2015)<\/strong><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-1057 size-large\" src=\"https:\/\/www.neolifesalud.com\/wp-content\/uploads\/NL-presion-arterial-3.jpg\" alt=\"pressi\u00f3 arterial\" width=\"1024\" height=\"683\"><\/p>\n<p style=\"text-align: justify;\">L\u201fobjectiu d\u201faquest estudi va ser avaluar els beneficis del control intensiu de la pressi\u00f3 arterial. Per aix\u00f2, es van incloure prop de 10.000 persones amb pressi\u00f3 sist\u00f2lica igual o superior a 130 mmHg i amb alt risc cardiovascular, encara que sense diabetis tipus 2. Aquesta poblaci\u00f3 es va seleccionar espec\u00edficament perqu\u00e8 es preveia una major probabilitat d&#8217;esdeveniments cardiovasculars en un per\u00edode curt, cosa que facilitava avaluar els resultats cl\u00ednics.  <\/p>\n<p style=\"text-align: justify;\">Els participants es van dividir aleat\u00f2riament en dos grups:<\/p>\n<ul>\n<li style=\"text-align: justify;\">Tractament intensiu: l&#8217;objectiu era mantenir una pressi\u00f3 sist\u00f2lica per sota de 120 mmHg.<\/li>\n<li>Tractament est\u00e0ndard: es tractava els pacients per assolir una pressi\u00f3 sist\u00f2lica per sota de 140 mmHg.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">A l&#8217;inici de l&#8217;estudi, la pressi\u00f3 arterial mitjana dels participants era aproximadament 140\/78 mmHg. Els resultats de l&#8217;estudi de la pressi\u00f3 sist\u00f2lica mitjana van ser despr\u00e9s d&#8217;un any de tractament, de 121.4 mmHg al grup intensiu i de 136.2 mmHg al grup est\u00e0ndard <\/p>\n<p style=\"text-align: justify;\">El m\u00e9s interessant de l&#8217;estudi \u00e9s que va ser interromput abans del previst, a causa dels beneficis cl\u00ednics contundents observats al grup intensiu. La troballa m\u00e9s cridanera va ser la reducci\u00f3 en la mortalitat per totes les causes, incloent-hi no nom\u00e9s causes cardiovasculars, sin\u00f3 tamb\u00e9 malalties renals, accidents, su\u00efcidis i fins i tot homicidis. Aquest benefici va ser inesperat i va sorprendre molts experts.  <\/p>\n<p style=\"text-align: justify;\">Aquest estudi va ser un dels m\u00e9s contundents a demostrar que controlar de manera intensiva la pressi\u00f3 arterial sist\u00f2lica per sota de 120 mmHg pot salvar vides i reduir esdeveniments cardiovasculars, fins i tot en un termini relativament curt.<\/p>\n<p style=\"text-align: justify;\">L&#8217;estudi tamb\u00e9 ens recorda que condicions com la hipertensi\u00f3, el tabaquisme i els nivells elevats de apoB (un marcador de colesterol aterog\u00e8nic) acumulen dany amb el temps: La hipertensi\u00f3 perjudica mec\u00e0nicament l&#8217;endoteli vascular, el tabaquisme produeix un dany qu\u00edmic al mateix teixit i el colesterol LDL danyat, iniciant la cascada ateroscler\u00f2tica. Per aix\u00f2, reduir preco\u00e7ment TOTS aquests factors de risc t\u00e9 un efecte acumulatiu enorme al llarg del temps. <\/p>\n<p style=\"text-align: justify;\">Anys despr\u00e9s el 2021, l&#8217;estudi STEP a la Xina tornava a demostrar que el control estricte de la tensi\u00f3 arterial disminueix els esdeveniments cardiovasculars i la mortalitat cardiovascular. <\/p>\n<p style=\"text-align: justify;\">Potser encara sentim o tinguem la creen\u00e7a antiga que tenir una tensi\u00f3 arterial de 140\/90 est\u00e0 b\u00e9, per\u00f2 despr\u00e9s de totes aquestes dades esperem haver-los obert els ulls i veure que NO \u00e9s aix\u00ed, cal canviar el xip i fer un control intensiu amb un objectiu de control TAS 120\/80.<\/p>\n<p style=\"text-align: justify;\">I \u00e9s que ja des dels anys 60, amb les primeres dades de l&#8217;estudi Framingham Heart Study, es coneixen els efectes perjudicials de la pressi\u00f3 arterial elevada (en aquella \u00e8poca es considerava hipertensi\u00f3 qualsevol valor superior a 140\/90 mmHg). Reduir la pressi\u00f3 des de xifres superiors a aquest llindar ja mostrava beneficis clars. <\/p>\n<p style=\"text-align: justify;\">M\u00e9s recentment, altres metaan\u00e0lisis han confirmat aquestes troballes. En persones entre 40 i 70 anys, per cada augment de 20 mmHg a la pressi\u00f3 sist\u00f2lica o 10 mmHg a la diast\u00f2lica, es duplica el risc de mort per infart, ACV o malalties vasculars. Aix\u00f2 no es refereix nom\u00e9s a m\u00e9s incid\u00e8ncia de malaltia, sin\u00f3 a m\u00e9s probabilitat de mort. Per exemple, tindre una pressi\u00f3 de 140\/90 en lloc de 120\/80 duplica el risc de mort vascular.   <\/p>\n<p style=\"text-align: justify;\"><strong>Altres \u00f2rgans afectats<\/strong><\/p>\n<p style=\"text-align: justify;\">I no nom\u00e9s el cor \u00e9s sensible a la HTA. El cervell \u00e9s particularment vulnerable a la pressi\u00f3 arterial alta, ja que tots dos depenen d&#8217;una bona perfusi\u00f3 en gots petits. La hipertensi\u00f3 \u00e9s una for\u00e7a mec\u00e0nica que fa malb\u00e9 aquests gots, i per aix\u00f2 augmenta el risc d&#8217;ACV i dem\u00e8ncia.  <\/p>\n<p style=\"text-align: justify;\">L&#8217;assaig SPRINT-MIND, una branca de l&#8217;SPRINT centrada en deteriorament cognitiu, va avaluar si el tractament intensiu podia prevenir la dem\u00e8ncia. Amb m\u00e9s de 9.000 participants, els resultats van ser una reducci\u00f3 absoluta del risc de dem\u00e8ncia en un 0,6% i una reducci\u00f3 relativa: 16%. Per tant, controlar la pressi\u00f3 arterial protegeix el cervell.  <\/p>\n<p style=\"text-align: justify;\">Els ronyons s\u00f3n un altre \u00f2rgan extremadament sensible a la pressi\u00f3 elevada. Tot i representar nom\u00e9s 1\u20132% del pes corporal, reben entre 20\u201325% de la despesa card\u00edaca (que \u00e9s la quantitat de sang que el cor bomba cada minut per portar oxigen i nutrients a tot el cos). Aix\u00f2 implica una xarxa vascular altament especialitzada i vulnerable. La hipertensi\u00f3 accelera el deteriorament de la taxa de filtraci\u00f3 glomerular (GFR) i disminueix la funci\u00f3 renal. La situaci\u00f3 empitjora si tamb\u00e9 hi ha glucosa elevada, una combinaci\u00f3 comuna, per exemple, a la s\u00edndrome metab\u00f2lica (que inclou hipertensi\u00f3 eresist\u00e8ncia a la insulina). Aix\u00f2 porta a una caiguda progressiva i perillosa de la funci\u00f3 renal, sovint sense ser detectada si nom\u00e9s es mesura la creatinina. Per aix\u00f2, a Neolife, davant la sospita de deteriorament de la funci\u00f3 renal fem servir cistatina C, que \u00e9s un marcador m\u00e9s sensible.<\/p>\n<p style=\"text-align: justify;\">El missatge principal d&#8217;aquests estudis \u00e9s que reduir la pressi\u00f3 arterial \u2013fins i tot en persones diagnosticades amb hipertensi\u00f3\u2013 pot reduir significativament el risc de malalties card\u00edaques, accident cerebrovascular i altres afeccions greus de salut. Tot i que l&#8217;ideal \u00e9s prevenir l&#8217;elevaci\u00f3 de la pressi\u00f3 arterial des del principi, els resultats d&#8217;aquests assaigs demostren que reduir la pressi\u00f3 arterial despr\u00e9s que ja ha superat els rangs \u00f2ptims ofereix enormes beneficis per a la salut cardiovascular. <\/p>\n<p style=\"text-align: justify;\"><strong>Ull: pressi\u00f3 arterial baixa, per\u00f2 no gaire. <\/strong><\/p>\n<p style=\"text-align: justify;\">Tot i que la pressi\u00f3 alta \u00e9s perillosa, una pressi\u00f3 massa baixa tamb\u00e9 pot causar problemes, encara que en aquest cas els s\u00edmptomes s\u00f3n m\u00e9s importants que els n\u00fameros. T\u00e8cnicament, es considera pressi\u00f3 baixa per sota de 90\/60 mmHg, per\u00f2 el que importa \u00e9s com se sent la persona. <\/p>\n<p style=\"text-align: justify;\">S\u00edmptomes comuns de pressi\u00f3 baixa inclouen mareig, atordiment, n\u00e0usees, desmais o s\u00edncopes, deshidrataci\u00f3 o set excessiva, manca de concentraci\u00f3, visi\u00f3 borrosa, pell freda i p\u00e0l\u00b7lida, respiraci\u00f3 r\u00e0pida i superficial, fatiga\u2026<\/p>\n<p style=\"text-align: justify;\">No totes les persones amb pressi\u00f3 baixa tenen s\u00edmptomes. Algunes poden viure sense mol\u00e8sties amb pressions com 100\/70 mmHg, mentre que d&#8217;altres no toleren b\u00e9 aquest nivell, especialment si han baixat de pes o estan prenent medicaci\u00f3 antihipertensiva. <\/p>\n<p style=\"text-align: justify;\">Per aix\u00f2, el maneig de la pressi\u00f3 arterial requereix un enfocament personalitzat. En perdre pes, fer exercici o canviar altres factors, la pressi\u00f3 pot baixar naturalment, i els medicaments s&#8217;han d&#8217;ajustar amb compte per evitar efectes adversos com ara desmais o p\u00e8rdua d&#8217;equilibri. <\/p>\n<p style=\"text-align: justify;\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-1057 size-large\" src=\"https:\/\/www.neolifesalud.com\/wp-content\/uploads\/NL-presion-arterial-4.jpg\" alt=\"pressi\u00f3 arterial\" width=\"1024\" height=\"683\"><\/p>\n<p style=\"text-align: justify;\"><strong>Variacions de la TA al llarg del dia i amb l&#8217; exercici.<\/strong><\/p>\n<p style=\"text-align: justify;\">Al llarg del dia, la pressi\u00f3 arterial fluctua de manera natural. Una observaci\u00f3 clau \u00e9s que durant la nit hauria de disminuir entre un 10% i un 20% en comparaci\u00f3 dels valors di\u00fcrns. Aix\u00f2 \u00e9s degut, en part, a la posici\u00f3 horitzontal del cos mentre dormim, cosa que redueix l&#8217;esfor\u00e7 que el cor necessita fer per enviar sang al cervell. A m\u00e9s, durant la nit hauria de disminuir el to simp\u00e0tic (el sistema d&#8217;alerta) i augmentar el to parasimp\u00e0tic, especialment el gandul, cosa que afavoreix la relaxaci\u00f3. En realitzar monitoratges continus de la pressi\u00f3 arterial es busca precisament aquesta reducci\u00f3 nocturna com un marcador de salut cardiovascular.    <\/p>\n<p style=\"text-align: justify;\">L&#8217;estr\u00e8s tamb\u00e9 hi juga un paper important. Esdeveniments estressants transitoris poden elevar significativament la pressi\u00f3 arterial. Deixar sense control l&#8217;exc\u00e9s de cortisol (hipercortisol\u00e8mia) \u00e9s tan perjudicial com la hipertensi\u00f3 persistent.  <\/p>\n<p style=\"text-align: justify;\">\u00c9s completament normal que la pressi\u00f3 sist\u00f2lica augmenti durant l&#8217;exercici, perqu\u00e8 el cor necessita bombar m\u00e9s sang per portar oxigen als m\u00fasculs. En canvi, la pressi\u00f3 diast\u00f2lica sol mantenir estable o baixar lleugerament. Aix\u00f2 passa perqu\u00e8, durant l&#8217;exercici, els vasos sanguinis dels m\u00fasculs es dilaten (vasodilataci\u00f3), cosa que redueix la resist\u00e8ncia a la circulaci\u00f3. Gr\u00e0cies a aquest mecanisme, l&#8217;exercici actua com a regulador natural de la tensi\u00f3 arterial: millora l&#8217;elasticitat de les art\u00e8ries, redueix la resist\u00e8ncia vascular i amb el temps ajuda a controlar la pressi\u00f3 en rep\u00f2s.   <\/p>\n<p style=\"text-align: justify;\"><strong>Hipertensi\u00f3 prim\u00e0ria vs. hipertensi\u00f3 secund\u00e0ria<\/strong><\/p>\n<p style=\"text-align: justify;\">Una altra cosa que deuen con\u00e8ixer \u00e9s que la hipertensi\u00f3 pot ser deguda a una causa i s&#8217;anomena hipertensi\u00f3 secund\u00e0ria. \u00c9s aquella causada per una condici\u00f3 m\u00e8dica subjacent que, potencialment, es pot corregir. S&#8217;estima que un 10% dels casos d&#8217;hipertensi\u00f3 tenen una causa secund\u00e0ria, per aix\u00f2 \u00e9s important no assumir que tota hipertensi\u00f3 \u00e9s \u201cessencial\u201d o prim\u00e0ria.  <\/p>\n<p style=\"text-align: justify;\">Hi ha senyals que poden indicar que \u00e9s una hipertensi\u00f3 secund\u00e0ria, per exemple, quan no respon als medicaments, si deixa de respondre a tractaments pr\u00e8viament efectius, si la pressi\u00f3 \u00e9s extremadament alta (per sobre de 180 mmHg) o si apareix de manera sobtada. Tamb\u00e9 \u00e9s sospit\u00f3s si alg\u00fa jove, sense antecedents familiars ni factors de risc, presenta una pressi\u00f3 alta. <\/p>\n<p style=\"text-align: justify;\">Altres causes secund\u00e0ries que no s&#8217;han de passar per alt inclouen malalties renals, estenosi de l&#8217;art\u00e8ria renal, trastorns tiro\u00efdals, apnea del son i l&#8217;hiperaldosteronisme (que pot ser degut a l&#8217;\u00fas prolongat de corticoides oa problemes a la gl\u00e0ndula suprarenal) entre d&#8217;altres.<\/p>\n<p>Des de Neolife creiem que maneig de la pressi\u00f3 arterial \u00e9s un dels pilars m\u00e9s importants per a la longevitat.
